    Pathophysiology[edit]
    • Adhesions form as a natural part of the body’s healing process after surgery in a similar way that a scar forms. The term "adhesion" is applied when the scar extends from within one tissue across to another, usually across a virtual space such as the peritoneal cavity. Adhesion formation post-surgery typically occurs when two injured surfaces are close to one another. This often causes inflammation and causes fibrin deposits onto the damaged tissues.[2] The fibrin then connects the two adjacent structures where damage of the tissues occurred. The fibrin acts like a glue to seal the injury and builds the fledgling adhesion, said at this point to be "fibrinous." In body cavities such as the peritoneal, pericardial and synovial cavities, a family of fibrinolytic enzymes may act to limit the extent of the initial fibrinous adhesion, and may even dissolve it. In many cases however the production or activity of these enzymes are compromised because of injury, and the fibrinous adhesion persists. If this is allowed to happen, tissue repair cells such as macrophages, fibroblasts and blood vessel cells, penetrate into the fibrinous adhesion, and lay down collagen and other matrix substances to form a permanent fibrous adhesion. In 2002, Giuseppe Martucciello's research group showed a possible role could be played by microscopic foreign bodies (FB) accidentally contaminating the operative field during surgery.[3] These data suggested that two different stimuli are necessary for adhesion formation: a direct lesion of the mesothelial layers and a solid substrate (FB).
While some adhesions do not cause problems, others can prevent muscle and other tissues and organs from moving freely, sometimes causing organs to become twisted or pulled from their normal positions.
    ------------------------------------------------------------------------------------------------------------------------------------
    Abdominal adhesions[edit]
Abdominal adhesions (or intra-abdominal adhesions) are most commonly caused by abdominal surgical procedures. The adhesions start to form within hours of surgery and may cause internal organs to attach to the surgical site or to other organs in the abdominal cavity. Adhesion-related twisting and pulling of internal organs can result in complications such as abdominal pain or intestinal obstruction.

Small bowel obstruction (SBO) is a significant consequence of post-surgical adhesions. A SBO may be caused when an adhesion pulls or kinks the small intestine and prevents the flow of content through the digestive tract. It can occur 20 years or more after the initial surgical procedure, if a previously benign adhesion allows the small bowel to spontaneously twist around itself and obstruct. SBO is an emergent, possibly fatal condition without immediate medical attention.

According to statistics provided by the National Hospital Discharge Survey approximately 2,000 people die every year in the USA from obstruction due to adhesions.[4] Depending on the severity of the obstruction, a partial obstruction may relieve itself with conservative medical intervention. However, many obstructive events require surgery to lyse the offending adhesion(s) or resect the affected small intestine.

    Abdominal surgery[edit]
A study showed that more than 90% of people develop adhesions following open abdominal surgery and 55–100% of women develop adhesions following pelvic surgery.[12] Adhesions from prior abdominal or pelvic surgery can obscure visibility and access at subsequent abdominal or pelvic surgery. In a very large study (29,790 participants) published in British medical journal The Lancet, 35% of patients who underwent open abdominal or pelvic surgery were readmitted to the hospital an average of two times after their surgery due to adhesion-related or adhesion-suspected complications.[13] Over 22% of all readmissions occurred in the first year after the initial surgery.[13] Adhesion-related complexity at reoperation adds significant risk to subsequent surgical procedures.[14]

Certain organs and structures in the body are more prone to adhesion formation than others. The omentum is particularly susceptible to adhesion formation; one study found that 92% of post-operative adhesions were found in the omentum.[15] It appears that the omentum is the chief organ responsible for "spontaneous" adhesions (i.e. no prior history of surgery). In another study, 100% of spontaneous adhesion formations were associated with the omentum.[2]

Ok enough with the technical side of things.
Many of you already know who I am but I'll give a little back story for them that don't.
2011 Jan 7th 3:37 AM with high fever and belly pain enough to make me scream off to the local hospital we go. Diverticulitis is the diagnosis and we're gonna cut you open NOW !!
1st surgery is just a 5 inch cut at the waist line and I'm home in 3 days.
30 days later it fails and I'm back under the knife.
This one is the full Monte with a Colostomy bag and drains and I can't see my feet for the mass of bandages.
10 days later and 50 pounds lighter they send me home.
My surgeon wants to do the reconstruct ASAP but I look and feel like a death camp survivor and opt to wait a couple of months to build up some physical reserves before another round of hospitalization.
60 days later they knock me out for the rebuild.
So except for the 20 inches of scar that runs from hip to hip bone and up to my navel in an upside-down T and missing most of my large intestine (Colon) it all works and I look normal enough and no Colostomy bag so that's a great thing.
    The Downside.
While I can and do have a full range of motion I don't dare use a large part as the payment later is just not worth the cost. To many squats, Deep knee bends. Simple bending over to pickup dropped keys. I never know just what will trigger an event but to many of any number of motions can definitely cause one. Garden to long. Pants to tight is an all to common one. I really need a good set of grandpa's suspenders and through away my belts.
Events come in several types and can range from 1 to 10 on the pain scale.
The 10 is always at the former stoma site and it feels like a knife being slowing driven in until it's stopped by the back ribs then the knife is twisted round in a clockwise rotation a few times and then left to hang in the hole.
It's all in my head right ?? HAH. Bullshit. This HURTS !!
The other most common event is a general gripping sensation like someone has a handful of my intestines and is squeezing and twisting at the same time. More a 7 in pain but it'll have me spring out of my chair involuntarily as it's an electrical shock feeling more then pain.
I was having sensations even before I was sent home and getting home returned to my usual smoking routine of doobies only to find they relieved my adhesion issue quite nicely.
Time passes and I develop a lung infection and can't smoke my much needed medicine. I'd read BadKats massive thread a couple times already so I knew the basics of edible oil so I nuked a fat bud in some cooking oil and wham bam I was stoned, in relief and couldn't be happier. That started my journey into oil and it's continued to the current date.

All it took was transplanting 14 veggers from the shed to the ground site to flower. I was so careful and limited the number of up and downs but still had a crappy night with that old gripping pain-discomfort and gobbled hash caps every 3 hours all night and it has left me stupidly whacked this morning.
Had more ultrasound of the general area yesterday and should meet with the specialist for interpretation next week. Still feels like I've been sucker punched in the guts.

  4. I hope the specialist can do something to help you with your pain. It's so frustrating.

The last MRI was with contrast but they still say it's nothing they can see on either MRI or the repeated Ultra sound scans.
It ( the pain) and I have reached an uneasy stalemate for the time being. I limit what I do through the day and Mr. Pain doesn't come a calling. Not what I'm happy with as I'm an active soul and love to garden and just be doing stuff all day long.

  6. I've always thought of it as like living with a dragon inside you. Most of the time it sleeps, and we tiptoe around and move gently to keep it that way, But every once in a while it wakes up and takes another chomp.

  8. Been a while since I updated this page. Haven't had a really bad event till last night. 3 hours in nasty dog slow traffic and I think it was the blasted seatbelt pressing on it for so long that had me screaming.

I'd eased off to a capsule a day and the 2 I took last night sent me zooming off to a point past Pluto but stopped the pain after 90 minutes.
    God Bless Cannabis !!!

  9. Woke up at 2am in serious agony. 2 caps and a codeine and suffered for 90 minutes before it finally eased enough for me to catch my breath. Didn't wake up till almost 9am and I'm still tender and moving slow.
No idea what triggered this one but it was in the top 5 of events. I almost woke the other half and had em take me to the emergency room.
    GRRRRRRRRRRRRRrrrrrrrrrrrrrrrrrrrrrrrrrrrrr Damn this shit.

20 minutes of the ultrasound tech pressing on the general area of the old Stoma site at 9am and by 1pm I'm in enough pain to chew off my own abdomen. FUCK THIS SHIT !!!.
3 of the RSO capsules I just made and a whole Vicodin and I'm still gritting my teeth to keep a lid on it..

Edit:: 3pm and I'm stoned as can be but still got a fire down below. Took the screaming edge off it and I'm creeping if I have to move.

  13. Once again I had a "Close Encounter of the Cannabis Kind"
3 of the newly made RSO caps at 4pm as I was still feeling a few twinges from the old Stoma site after my transplanting adventures.

I was fine for a couple hours until just about exactly the last bite of the fabulous Del Taco dinner we were having. 6pm or so.

Suddenly Mr. Velvet Sledgehammer snuck up and Whacked me upside the bloody head to where it was lay down or fall down. I'm stoned off my ass and have the overwhelming urge to pee.

Even then I was OK. Keep thinking I just took a bit to much and it'll ease up in a bit. NOPE, nada, Kept building and building and I reached that moment when you think I really misjudged the dose and I'm in serious trouble. And over riding it all was the urge to take a whizz. But I can't. It's just urge and it is really a pressing I'm going to pee myself URGE!

Vision went tunnel, Audio was way to loud, Skin had the crawlies, Hot flushed then freezing cold the next moment.
I still need to PEE :)

Spins came on and I thought I was going to lose my dinner when the room started to get cranked up and I had to hold on as the damn planet tried it's best to through me off.
GOD let me just PEE !!

Now I really don't feel well. My belly hurts and I'm FUCKING Miserable and have been for 3 hours now. Keep biting my tongue and chanting to myself " I will NOT call 911 and report a Cannabis overdose. NOPE !!!"
I still have to FUCKING PEE !!!

Somewhere around 1 am in flat out exhaustion I passed out and didn't wake till 6am.
WHEW !!!!! Mother of God what a Ride.
Damn I think this one gets moved up to the #1 slot of Wow did I ever misjudge that dose.

  16. Damn that does not sound like fun. I’m having abdomen issues again at the moment too. Chronic dehydration due to only having half a colon, leads to slow bowel function a couple times a month. Add to that constant bacterial infection from an abscess in my pelvis that nobody can seem to figure out and a viral infection on top of that, and the last few days have been awful. I’m debating taking another capsule or not to help with the pain, but your experience has me second guessing if I want to feel that on top of everything else. Fuck it, I can drool the rest of the day....

  17. Snowed in at the Cabin. ( a semi regular event ) We got about 30 inches and after 3 days I'd burned all the easy to get at wood. It's about 30 feet uphill to get at the main wood pile and 20 minutes of combined shovel and stomping got a path knocked down and I started hauling in wood.

6 trips and I felt something tug. Finished the 7th load and I couldn't get the snow boots off without whining. It's not in the usual one of two places I get a bite your hand to keep from screaming pain from. This is higher on the right side where one of the drain scars is. Odd location but mother please make it stop, knife in the ribs, pain hit.

The last RSO capsule and a Hashy with 3 aspirin and a horded codeine before it really let up.

I see it's been a year since I was in enough pain to update and as the old proverb goes.
"May you live in interesting times"

  18. That sucks BNW. I hope your medication gives you long lasting relief.

  19. It generally does Tim. It's that 45 minutes of agony until it kicks in. Thankfully smoking does take the edge off about 30% so I toke until the capsules take full effect.
