definately dark souls, i bought it, and gave up very quickly; its just too hard of a game for me, i couldn't even get past the first part.
There can be only RNG.

Just watched this gameplay vid for the new MGS. Seems pretty challenging if your going for the no alerts, no indicators, silent assassin type of gameplay. I have a feeling this guy playing the game is making it look easier than it really is.
Also, those damn prometheans in Halo 4 on legendary gave me a hard time. I replayed a few of those missions over and over before figuring them out.
Be a lot easier for me to answer: Which games do you wish had great AI?
In my experience most games' difficulty is from the skill difference against enemies in early game...not necessarily because the AI had superior tactics.

The enemies are hard, meaning that can kill you quickly, but not necessarily intelligent.
For instance, you can cheese lots of enemies into falling off ledges, into holes etc, by just manipulating their aggro paths, or get enemies like bosses to fall into loops of certain attacks that can be exploited.
I always found games that relied on stealth to usually have good enemy AI. Games like Metal Gear, or Deus Ex.
the fact that you think fear has bad ai means your opinion can be recycled... FEAR is widely accepted as one of the games with the geat AI

This generation hasn't had the greatest AI. Developers too focused on prioritizing graphics over AI.
The best I've seen is still STALKER Shadow of Chernobyl from 2007...

Elder Scrolls Online has pretty smart enemy AIs.
EDIT: That game is gonna be fucking awesome.
Splinter Cell Conviction has a good AI when I play it on Realistic,I think shooters tend to have the best AI IMO

Far Cry 3 AI was fun. Skyrim AI was a bit ridiculous at times but the dragons could fuck you up if you didnt get them on the ground.
Metal Gear, Uncharted, Crysis..... Having good AI can make or break a stealth game.
