Question about documentation

  1. Hello,

    I was just wondering about the documentation that I must bring during my appointment. I'am experiencing pain in my left and right arms(particularly the right)that has over the years as they have been injured more turned into chronic pain that has come to a level where it is now effecting my training. I have medical documentation for my most recent inury when I broke my right medicarpal. However all previous injuries were treated by my father(A licensed orthopedic hand surgeon for which I dont have documentation of). What should I do in this situation? Should I have my father draw up some method of documentation or a letter for these past injuries. Any help would be greatly appreciated.

  2. A letter from him, if he is still licensed in the state you are applying, will work with your recent records. You should qualify under chronic pain.
