  1. So I moved to CO about a year ago and I'm about to see a doctor about a recommendation. My reasoning behind this is because a few years ago I broke my jaw and had to have a plate put in. Now every time it rains or snows/ during cold days/months I have pain in my jaw. Sometimes making it hard to eat anything at all.

What i am wondering is, would it be necessary to have my medical records from the surgery/ plate? Or will the doctor be able to tell that i do have a plate by feeling the difference in my jaw. I take tylenol typically 500-750mgs per day. I would like to have a liver by the time im 40 lol.

  2. It would be best to have your medical records with you but they're not necessary.

  3. is it necessary to wait a full year before you can get it or do you just need proof of address?

  4. you need proof of a valid condition...medical proof

get your medical records of the issue and all the 'treatments' you've tried for relief.

if you have proof, GREAT, go get that rec/card...if not, don't game the system.

  5. The only problem with getting my records is I moved out if state so they would have to send me a release form to sign then I have to send it back and wait for my records
