kgb glass makes donuts similar to those, i'm not sure if thats one of theirs though. You can google them to find more info
I went to their website before I made this post, but there was nothing to buy on the website. The contact information was also only for wholesale or to tell them how much you liked the piece you already had. That's why I figured if I could find someone locally who could make me one.
They might not have anything on their site but they do have a list of online retailers that carry their stuff. It doesn't seem like you're getting many responses here so you might want to look into ordering from one of them maybe.
