I can't say exactly what the problems are, but I can make some observations:
1) It looks like you have water tubes going directly into the rockwool cubes. If it's possible to overwater with a DWC, that's how to do it. Personally I just set them up so the bottom of the net-pot barely touches the nutrient solution, and that wicks up plenty of water for the plants. They will put down roots pretty quickly.
2) When I do my grows, I bury the rockwool in the hydroton. That puts the roots closer to the water and provides more hydroton for the roots to latch on to as they grow out of the cube.
3) It's hard to tell, but it looks as though there is something wrapping the sides of the rockwool cubes? If that's true, remove this wrapping. The roots will grow out of the sides of the cube in addition to growing out of the bottom.
4) If you're doing any kind of foliar feeding, I'd lay off of it for a while.
5) What are you doing for aeration? Are there bubblers in the bottom of that tub?
