4in holes on both sides of the bottom for air intake(inserted a 6in long 4in PVC into each hole to keep light from escaping)
Also, Should the 90cfm fan keep a 6ftx4ftx2ft box cool enough?

anything vented will work well, as long as air is being drawn over the bulb your a warm happy clam. (your only warm if you vent the exhaust into your house / room)
now with the dimensions you gave, your room is 48 Cubic feet, with a 90 Cubic Foot /Minute fan that should be fine, although after you have everything running you might decide differently.
If you have problems with flow try making a dedicated intake to the grow room. if you have already done that then try adding a booster fan to the intake. (200cfm range)
how warm you box is really depends on your intake temps.
I myself have an intake on a thermostat that draws air from outside when my room goes above 74 degrees.
