I have the exact same prob. My grow is spotless, no bugs, nothing. The only odd thing i've ever noticed was that on two separate occasions a fan leaf had water sitting on its surface. I don't water the leaves, so my best guess is condensation. My humidity sits between 50 and 60%. I've no idea if this would cause the holes though.
